Transition my care

Need help selecting an Aspirus Health Primary Care Provider? Call the Aspirus Welcome Center at 833.811.4176.

The Welcome Center can assist you with:

• Transferring your medical records to Aspirus Health.
• Obtain personal health information, such as medical history, allergies, medications and immunizations.
• Sign you up for MyAspirus, our online portal where you can view portions of your medical record, schedule appointments, pay bills and request prescription refills.
• Help you select a Primary Care Provider (PCP) and schedule an appointment.

Transition of Care Form

If you or a member of your family are undergoing medical treatment or are pregnant, please fill out this form and send it to us within 30 days of your new enrollment date. Complete one form for each family member, as needed.
