WELCOME -- PLEASE READ THIS AGREEMENT CAREFULLY BEFORE ACCESSING OR USING OUR INTERNET SITE.  If you agree to the terms listed below, click on 'I Accept' at the bottom of the document. The terms 'us' or 'we' in this document mean Aspirus Health Plan.

'This Site' means Aspirus Health Plan’s on-line information system. Through the Site you can view certain information about your medical benefits, and information about the claims that health care providers submit for services they have provided to you or to your enrolled dependents.  Your benefits are contingent on all the applicable terms, conditions, limitations and exclusions of your benefit document.  Information you can access through this Site includes dates of service, provider charges, provider discounts and claims status.   It does not include procedural or diagnostic information about the services received.  

The information we do provide is given to you to assist you in managing your medical benefits. The Health Insurance Portability and Accountability Act of 1996 (HIPAA) imposes certain rules designed to protect the privacy of an individual’s personal health information defined by HIPAA as 'Protected Health Information'('PHI').   It is our intent to provide information through this Site, as allowed by HIPAA, to you as the subscriber who is responsible for payment under your health plan.  The subscriber is typically the covered employee or in certain cases the principal member or contract holder.  You, as the subscriber, agree not to help anyone else access our Site if they are not authorized to do so.  You also agree to not introduce a computer virus into this Site or to do anything else that would damage this Site.

We take measures to provide you with accurate information.  If you believe that any personal information we have about you or your dependents is not accurate, please let us know by contacting Customer Service at 1.866.631.5404 from 8:00 a.m. to 5:00 p.m. Monday through Friday, excluding federal holidays.  It is not our practice to include personal or account information in standard emails that we may send to you over the Internet.  To respond to you about personal matters, we may require communication by telephone or in some other manner.  Although this process may be inconvenient, it is done to protect sensitive information.

You will also find cost information regarding providers and clinics on the Site. The information set forth regarding costs for providers and clinics is intended to be informational only and Aspirus Health Plan does not guarantee the accuracy of the information reported nor does Aspirus Health Plan endorse any particular person, clinic or provider group.  Your actual individual cost may vary from the costs listed.

We understand that the security of PHI is important to you and our security measures include a complex set of interacting network, application and operating system safeguards including: Firewalls, Intrusion Detection, Alarms, Encryption, ID codes, Passwords, Secure Messaging, Audits and Tests.

On the Internet, we take a number of measures to authenticate your identity when you access our services.  We also take action to protect sensitive information while it travels the Internet to and from your computer.  Yet, even with our best efforts, 'perfect security' does not exist on the Internet.  You can help protect your information by protecting your Password and logging out of our Site as soon as you are finished.

We will not be liable for any delay, difficulty in use, incompleteness of information, computer viruses, malicious code, loss of data or compatibility issues that you may encounter while using this Site.  You use this Site at your own risk.  We are not liable for any direct or indirect damages arising out of your access to our Site, or any other link we provide to another site.  You assume full responsibility for using this Site, which is provided without warranty of any kind, express or implied.

Aspirus Health Plan reserves the right to change this agreement at any time by revising the terms and conditions herein. I will regularly review these terms and conditions and agree that continued use of this Site following any changes will constitute my acceptance of such changes.
